Life Insurance
Group Term Life for members and spouses under
age 60. Minimum of $50,000 to maximum of $250,000 in increments of
$10,000. Also can include child coverage, waiver of premium, AD&D
benefit, conversion privilege and accelerated living benefit.
Group Level Term Life for members and spouses between ages 18
and 70. Benefit amounts from $50,000 up to a maximum of $150,000. 10
year term renewable to age 95. $5,000 to $10,000 available
per child. Simplified underwriting.
Senior Term Life is a guaranteed issue, level benefit life
insurance plan designed to cover final expenses for members and their
spouses ages 45 through 75. Termination is at age 100. Minimum of
$2,000 to maximum of $25,000.
Student Term Life is an individual policy sold as a
cross-sell to student accident coverage in amounts of $10,000. At age
25 the policies terminate and convert to a "conversion"
policy (see below).
Conversion Life is a whole life paid-up at 65 policy and is
automatically offered to policyholders with
Student Term Life coverage at age 25.
Juvenile Life (SecureStart®) is an individual term life policy that
automatically converts to whole life. Simplified issue from $10,000 to
$20,000.
Accident
Accidental Death & Dismemberment free and
voluntary coverage. $1,000 to $2,000 free coverage provided by
group with voluntary face amounts up to $250,000. Numerous product
(riders) and price variations driven by the group/client.
Student Accident covers students and interscholastic athletes
for accidents only and can be written on a blanket or voluntary participation
basis. Coverage offered includes K-12 school time, nursery school, college,
special risks sports, 24-hour, dental accident and term life.
Health Insurance
Cancer Indemnity insurance is available to
members, spouses and children who are cancer free (usually for a period
of 5 years prior to application). Provides a daily hospitalization
benefit for cancer and specific benefits for riders such as ambulance
and skin cancer. Numerous product (riders) and price variations are
available and variations exist by group/client. Benefits reduce by
half at age 65. There is a $300,000 lifetime maximum per
insured.
Executive Health Program is a distinctive supplemental health plan that combines next generation
medical reimbursement with care navigation and guidance. The program provides
preferred patient status with world renowned physicians, resources to help
manage medical conditions, preventive care to achieve better health and
protection during travel and medical emergencies. With this solution, companies
can enhance their benefits package on a selective basis to aid in recruitment
and retention.
First Diagnosis cancer insurance is available to
members, spouses and children. Positioned as first diagnosis, the plan
has a low daily hospitalization confinement benefit of $50 or $100 and a large
one time benefit of $5,000 or $10,000.
Hospital Indemnity Protection provides daily
benefits from $50 to $200 per day including an array of riders (i.e.
cancer, accidental death, skilled nursing, etc.) and benefits
available to members, spouses and children. Terminates at age 65.
Numerous product (riders) and price variations driven by the
group/client. Accident Only Hospital Indemnity is also
available.
Medical Expense insurance provides affordable and essential voluntary health care
coverage to employers for their employees. The main product offering is a
Limited Benefit health product Employers can choose from a fixed indemnity
or expense-incurred plan depending on their size and needs. Within each plan
type there are five plan options to choose from with varying degrees of coverage
and the option of including an employee's family. Ancillary products like Life,
Short Term Disability and Dental can also be offered
Pre-65 Retiree Medical insurance was developed to effectively reduce the
employer's Retiree healthcare expense and liability while providing retirees
quality health coverage. Employers may select from three plan options paying
for covered inpatient and outpatient services and prescription drugs.
Health and Accident
Student Health and Accident covers college students for health and accident.
Every college plan has its own individualized set of benefit levels. Most plans
having a maximum of 35k to 50k.
Medicare Prescription Drug Insurance
Employer Part D group options are available to employers in conjunction with our Retiree Medical programs. Our nationally recognized PDP partners and Silverscript provide these Part D options.
Part D Plan multi-plan Medicare prescription drug insurance and decision tool program provides members and spouses of participating groups a one-shop-stop for their Medicare Supplement and Part D coverage. We have contracted with Kelsey National as a general agent to market plans from national PDP carriers Cigna, Caremark, Aetna and UAFC.
Medigap
Medicare Supplement offers national coverage and standardized plans available to member and spouse. Primarily targeted to those turning age 65
(Medicare eligible age).
Retiree Medical is a sponsored employer
health plan with flexibility in plan design and contribution level
(very similar to Medicare Supplement benefits but also has ability to
provide prescription drug indemnity plan and mail order
service).
Travel Insurance
Travel Protection Plans for retail and wholesale (i.e. tour operators,
cruise lines, and other wholesale buyers). Coverage includes up to $10,000 of
trip cancellation/interruption, accidental death of up to $25,000 (common carrier
up to $100,000), and coverage for other medical, emergency and baggage expenses
as well as medical emergency assistance.
